For gear, we're focusing on a high holy damage negation build.

We're going to boost this equipment selection with a few talismans:
The Haligdrake Talisman 2 significantly increases the negation of holy damage,
The Dragoncrest Shield Talisman 2 greatly increases physical damage negation,
And the Greatshield Talisman boosts your guarding ability, used in tandem with my very basic Golden Beast Crest Shield.

As an optional extra layer of protection, I use Lord's Divine Fortification, greatly increasing holy damage negation for myself and my allies, and Golden Vow for increased attack and defense for myself and my allies.

All of this together ends up giving VERY, VERY decent protection during both phases of this fight and will allow you to suffer less from any mistakes you may make.

The best weapons against Radagon are:

The first, my favorite for the first turn, is a maxed Morning Star with Royal Knight's Resolve and a Flame Art affinity, granted by the Red-Hot Whetblade.

The second option is the almost fully upgraded Blasphemous Blade or, as I like to call it, the Togetha Blade.

Finally, the weapon I used for most of my first playthrough was the fully upgraded Bloodhound's Fang.

As the 4th and 5th notable options on the weapon list, you can try the Scorpion Sting or the Rotten Crystal Sword to inflict Scarlet Rot on Radagon during the first phase.

And, as a very last resort, you can use other items like Exhilarating Aromatic, Blood Boiling Aromatic, or Exalted Flesh to increase your attack power.

Finally, the very last piece of our build for this fight is none other than the famous fully maximized Mimic Tear, Spirit Ash.

Now let's get to the fight.
Before entering the deafening portal, I burst the Lord's Divine Fortification, then the Golden Wish, and finally I drink my Flask of Wonderful Physique to enter the battle fully prepared, because I no longer have time to do something else once you've crossed that barrier.
At the end of the cutscene, it's time to summon the Mimic Tear and begin the fight.

This build will do a great job of helping you avoid getting crushed by Radagon's attacks, but it will serve you well to get used to dodging some of his attacks. Holy attacks are probably the most annoying, so remember that if you see him about to hit the ground with his hammer, jump, either on the spot or towards him to avoid the shockwave. The rest of his hammer blows are standard blows that can be avoided by dodging or blocking with your great shield.
Just keep an eye on his health dropping to about half as he will start teleporting everywhere, so be patient and wait for an opening to counterattack.

For the second phase, Radagon will transform into Elden Beast. This is the simplest phase, but the most boring, because there will be a lot of chasing and running to avoid damage and try to hit the boss. As you begin this second phase, you will likely need to use your incantations again to strengthen your character and the Mimic in order to do more damage and avoid additional damage from holy attacks.

The benefit of having Mimic Tear is that most of these attacks will usually focus on him, or if they do hit you, the incantations and gear you have equipped will either keep you safe or reduce a lot of the damage taken.

The best opportunities for you to attack are to avoid the Elden Beast's standard sword attacks, by dodging towards him. He typically does 3 swings followed by a belly flop, each of which is avoidable and provides a good opportunity to counterattack.
