It's important to note that fillable forms in Google Docs cannot be protected or restricted to form filling, which means users can inadvertently edit, modify, or even delete parts of the document. If you need to protect the form, you will want to convert it to PDF and add fillable PDF form elements, or create the form in another tool, such as Microsoft Word, Google Forms, or Microsoft Forms.
Additionally, unless you have Google Workspace, you won't be able to save your Google Docs form as a template. However, you can make a copy of the form file each time you want to fill out a new form.
