No icons on desktop, no Start menu or no fix in taskbar

The Start button, taskbar and desktop icons are missing. The problem is probably caused by the Windows Explorer shell. A file named explorer.exe failed to run. You need to manually tell Windows to start the process by going to Task Manager and running explorer.exe.
